The STRIDE Wellness Committee invites you to participate in the STRIDE Wellness Incentive Program. The program seeks to promote and support health awareness, individual responsibility for a healthy lifestyle, decreased risk of disease and enhanced quality of life. Your participation in the program is encouraged by increasing your allotted benefit dollars during the fiscal year.

As an eligible employee, you have the wellness year (May 1 – April 30) to complete the program’s requirements. Failure to complete the program requirements in a timely manner means you will not be eligible for the wellness dollars the following fiscal year (July 1 – June 30).

The Committee encourages you to read this overview and complete the necessary steps on your journey towards a healthier you, while earning extra benefit dollars.

Wellness Program Eligibility

All Cerro Gordo County Employees receiving benefits are eligible to participate in the STRIDE Wellness Incentive Program except when:
• The employee does not fulfill the program’s requirements during the wellness year, therefore forfeiting their eligibility for upcoming fiscal year.
• The employee is considered a Temporary or Seasonal position with Cerro Gordo County

Allotted Benefit Dollars + STRIDE Wellness Program

In 2015, Cerro Gordo County adopted a self-funded health insurance structure in which employees of Cerro Gordo County share in the cost of health insurance and health-related programs. As a result of the self-funding structure, the employees are also able to share in potential savings generated by low usage of designated funds from healthy employees. To foster this savings on investment and encourage healthy behaviors of employees, the STRIDE Wellness Incentive Program was established in which employees can increase the amount of his/her benefit dollars in the Fiscal Year (July 1 – June 30) by:
• Single Plan: $ 15.00 / month ($180 annually)
• Family Plan: $ 15.00 / month ($180 annually)

Participation in the Wellness Incentive Program

Participation in the STRIDE Wellness Incentive Program is voluntary. Please see the EEOC Notice on the STRIDE Portal for further clarification on personal health privacy information.

Program Requirements:

1. Complete Biometric Screening or Submit Measurements from Healthcare Provider
Blood panel consisting of cholesterol, triglyceride, blood sugar/A1C check, Blood Pressure Check, Body Mass Index (height and weight) measurement

Participants may send his/her biometric results obtained by his/her healthcare provider to the Health Promotion Team or use the Biometric Screening results performed by the Health Department. (New employees who miss the Spring Biometric Screenings may call 641.421.9312 to schedule appointment with the Health Promotion Staff upon hire.)

2. Complete a Health Risk Appraisal Questionnaire Online
A health risk appraisal is a series of health and lifestyle questions that offer insight into your risk for chronic conditions such as heart disease, diabetes and metabolic syndrome. Complete the questionnaire at time of enrollment at stride.cghealth.com

3. Earn Specified Amount of Wellness Points based upon Risk Stratification during the Wellness Year (May 1 – April 30)
Based upon an employee’s responses to the Health Risk Appraisal and his/her biometric measurements, employees may be required to complete various wellness activities to maintain eligibility in the Wellness Incentive Program and demonstrate efforts towards improving his/her health.

The following table illustrates the criteria for determining if an employee is at an elevated risk for chronic health conditions and the corresponding wellness activity points necessary to maintain eligibility in the wellness incentive program.

Submitting Wellness Points:

Employees will be asked to submit wellness point activities using the secure STRIDE Wellness Portal online via work or personal computer or other internet accessible device.

Wellness Incentive Program Retention:

To remain eligible for the Wellness Incentive Program, participants must complete all required activities and earn the specified amount of wellness points prior to the start of the new wellness program year (Deadline: April 30). The wellness year and the benefit year are not the same dates; this allows the Auditor’s Office time to set benefit packages appropriately.

Wellness Year: May 1 – April 30 (period to earn wellness points)
Incentive Benefit Year/Fiscal Year: July 1 – June 30th (period receiving extra benefit dollars)

Wellness Incentive Program Enrollment Process

STEP 1: Enroll in the Wellness Incentive Program by completing the Online Enrollment form found at stride.cghealth.com

STEP 2: Choose “YES” on the County Benefit Enrollment online form marked: Enroll in STRIDE Wellness Incentive Program

STEP 3: Complete the online Health Risk Appraisal located on the STRIDE Wellness Portal at stride.cghealth.com

STEP 4: Include your most up to date Biometric Screening values in your Health Risk Appraisal responses. stride.cghealth.com



Upon acceptance in the program, all participants will be sent quarterly communications to notify them on their wellness point status.
*The Health Department offers annual Biometric Screenings to all county employees in the Spring. Individuals who choose not to complete the Biometric Screenings with the Health Promotion Team can still be eligible for the Wellness Incentive Program by using Biometric Results obtained by their primary care providers.
